After speaking to some people this is my interpretation of how to
fill out the claim form.
Name & address - fill this out if something like your apt. # is
omitted in the supplied address - (make it clear for the post office).
Don't forget to include tel# and email.
1. Amount of claim - how much did you pay for the stock.
2. basis for claim - fraud, breach of fiduciary duty by the directors and officers of officers of the debtor and the debtor misrepresentation and claims under Federal Securities Laws.
3. Priority claim - Is it an IRA or retirement account (custodial
account) ? Check other and list the amount lost in amount.
4. attach trading history. (exactly what docs may be necessary can be discussed further) but you can also include your self prepared
summary.

First of all, thanks.
Regarding #1 - "how much did you pay for the stock." I am assuming this is the recommendation for those who are still holding their shares. I was actually going to put my exact loss (purchase amount minus sell amount since I sold on the day BK was announced).
Regarding #2 - Did an attorney or an accountant recommend stating fraud for this one? I am just wondering since fraud hasn't been proven yet.
Regarding #3 - I have no idea what number to put here. Possibly a number associated with each of us in the shareholder list which we should be listing here?
Regarding #4 - I am assuming this one does not relate to shareholders since we do not have a "secured claim." Do you agree?
Regarding the address you posted. In case anyone missed it, there was apparently a problem with the initial address given to us in the original proof of claim mailing. I do not have the letter with the updated address with me right now, so can you verify that the one you posted is correct? (for the sake of others reading this in case they didn't get the corrected address letter)
