Urgent :advice needed

please advise if you can

75 year old  New York (Westchester) woman has multiple special needs including limited mobility, impaired vision,  and has undergone surgery for brain tumor
Neither she or her family has any substantial income  and she has been denied assistance by all the usual channels.She and her family have explored several avenues to no end and time is running out at current residence .Her family cannot attend to  her needs in their own home.
There must be a Right Place for "Jane" to live, love and be loved and remain near her family.
(and have her needs met compassionately)
