It is a normal outcome with group dynamics even during a crisis, no one takes action as they are waiting for someone to lead. It is unconscious. This happens with car accidents, assaults, ect. People are afraid to get involved because they cannot assess their own safety. Everyone standing around watching and not acting, not even thinking to call emergency.

We can prepare ourselves by thinking about how we may act during a situation, but we never really know how we will behave. Part of it too has to do with how tired we may be, etc.m our judgement may be low at that particular moment/day.

Perhaps we are taught too much to follow instructions instead of lead...
