I live in Sweden. Here it's covered by the highest taxes in Europe - over 50 per cent goes to taxes. Then we are covered in many ways. My mum had surgery and that cost 80 SEK (around $13).

Health insurance never seems to really cover all the costs, its a bit of a waste of money.

No, I have no health inusrance. I choose not to. I am a naturalist and believe in pro-active care vs. reactive care. I (or my children) have not been to a doctor for an illness in over 5 years. I have studied and researched so many aspects of natural health that I feel confident that I can take care of any "symptoms" that do appear by addressing the underlying causes.

Yes, I have insurance but without monthly meds of $450. out of pocket, in one year I had $60,000. out of pocket and had to second mortgage my house. Of course I have pre existing problems at the age of 64 as almost anything you can name, they will say was pre existing just to not cover you. We lose lose.
